The Monessen Athletic Department will have Sports Physicals for the 2025-2026 school year on Friday, May 16, 2025, from 8:30 AM to 2:00 PM in the Nurse’s Office at Monessen High School on a first-come, first-served basis.  To be placed on the list, student-athletes must complete the following steps:

1. PIAA Physical Forms can be found in Ms. Naccarato’s Office or the High School Office.  The form can also be downloaded at https://monessenathletics.bigteams.com under Announcements.
2. Student-athletes must take these forms home and have their parent/guardian fill out all information on all pages, in their entirety.  Please make sure the parent/guardian signature and student-athlete signature, as well as all medical information, are provided on each page.  
3. Student-athletes must return the PIAA Physical Form to Ms. Naccarato.  Ms. Naccarato and Leigh Keller (Athletic Trainer) will review the PIAA Physical Form, and if everything is completed on the form, the student-athlete will be given a time slot for May 16, 2025.  Ms. Naccarato will let the student-athlete know their time slot.  
4. The PIAA Physical Form is good for one school year once the doctor signs and clears the student-athlete for participation. 
5. The only requirement for the PIAA Physical to be valid for the 2025-2026 school year is that the physical has to be completed after May 1, 2025.  Any physical completed before May 1, 2025, will not be valid for the 2025-2026 school year.  
6. Any student-athlete who does not get a PIAA Physical here on May 16, 2025, must get the physical on their own with their PCP or at MedExpress.  For the student-athlete to be cleared to participate, the PIAA Physical Form must be signed by the PCP or MedExpress. 
7. The completed PIAA Physical Form must be returned to Ms. Naccarato by Wednesday, May 14, 2025, at 2:00 PM to possibly reserve a time slot.
